What is Hospice
When Life Matters Most, pain and symptom management, as well as education and support are what patients and families need to focus on. That’s what really matters and is truly important. And that is what Hummingbird Hospice provides.
Our hospice team helps patients, and their families make the most of the time that remains, through a unique integrated healthcare approach, where the family and patient are the focus and not the disease.
Hospice is a team of specially trained health care professionals caring for the patient and relieving anxiety for the caregiver/family. Our support team helps with symptom management, social, emotional and grief issues that patients and families may experience. Hummingbird Hospice brings comfort, quality, and support to the remaining months, weeks, and days.

Who is Eligible for Hospice
Hospice care is for people who have a life-limiting illness that possibly can transition the loved one within six months, and have chosen to focus on comfort care over aggressive treatment.
Diagnoses commonly associated with hospice care include these and other terminal diseases:
• Heart Disease
• Respiratory Diseases
• AIDS/HIV
• Alzheimer’s/Dementia
• Liver/Renal Failure
• Neurologic Diseases
• Cancer
• Stroke or Coma
hummingbird Hospice works with the patient and family to develop a customized plan of care, specific to their disease and individual needs. Eligibility criteria includes:
• Frequent hospitalizations
• Progressive weight loss
• Deteriorating mental abilities
• Frequent infections
• Multiple falls in the last six months
• Treatment is more of a burden than a benefit
• Shortness of breath even while resting
• Spending most of the day in a chair or bed
• Increased medication to manage pain or symptoms
Needing help with:

getting out of bed

Where is Hospice Care Provided
Hospice services can be provided wherever the patient lives. Many patients are most comfortable in their own homes with their families. Hummingbird Hospice comes to the patient rather than move them to unfamiliar surroundings.

• Assisted Living / Group Home Facilities
• Nursing Homes

Who Provides Hospice Care
Hummingbird Hospice is comprised of an integrated clinical team designed to provide continuity of care to the patient. Based upon the customized individual care plan, each discipline brings the specific needs, wishes, and values chosen and developed with family’s involvement, to the declining loved one.
This comprehensive plan of care is directed by the patient and carried out by our support team, which is comprised of:
• Patient and Family
• Medical Director
• Experienced Registered Nurses – RN/LPN
• Certified Nursing Assistants – CNA
• Social Workers
• Chaplain/Bereavement Counselor
• Patient’s Physician
• Trained Volunteers
Hummingbird Hospice Provides A Teamwork Approach
Who Pays for Hospice Care
Hospice is covered under the Hospice Medicare Benefit, most Medicaid plans, and many private insurance plans. This means there is usually no out of pocket costs to the patient or their family. Most plans cover hospice care and the medications, medical supplies, and medical equipment related to the hospice prognosis.
Earlier referrals to hospice help patients and family's access and receive the maximum benefit and full scope of hospice services.
